Job SummaryThis position is responsible in a clinical care advisory role to assess members for wellness education and disease management; introduce company website tools to members to encourage personal health management; assist in program development/projects; educate members regarding wellness and specific conditions; facilitate the coordination of care for members; and may mentor staff.** This is a remote position doing telephonic outreach**
JOB REQUIREMENTS:
- Current, unrestricted license in the state of operations in one of the following: Registered Nurse (RN), or Licensed Professional Counselor (LPC), Registered Dietician (RD) or Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Exercise Physiologist
- 3 years clinical experience.
- 1 year case management or condition management experience in a managed care/health insurance company
- Patient education experience.
- Computer/system software experience.

- CSR Certification
- Renal Experience
- Bilingual English/Spanish
