Outpatient LPN

Hartgrove Hospital is seeking a full-time Licensed Practical Nurse to provide direct patient care under the supervision of a registered nurse and/or medical staff in our outpatient clinic settings. The LPN will work closely with our telepsychiatric medical staff to deliver aspects of direct patient care, including administration of intramuscular injections, and engage in patient support activities. This is a 40 hour per week position with a schedule of Monday through Friday, with periodic evening times required and duties at both the Oak Park and Chatham outpatient locations.

The primary responsibilities are as follows:

• Work closely with telepsychiatric medical staff to initiate patient appointments, take and record height, weight, vital signs from patient at appointment time, and relay behavioral observations to medical provider.
• Assist with medication reconciliation, answer patient questions about medications, and other nursing care activities as directed by medical staff.
• Administer intramuscular injectable medications as directed by medical provider.
• Provide administrative support to medical providers, including submission of prior authorizations, medication reconciliation with pharmacies, and outreach to patients as needed.
• Provide exceptional customer service over the phone and in person to all patients/families/guests.

Job Requirements:

• Must be a graduate of a LPN program.
• Must be licensed as a Licensed Practical Nurse in the state of Illinois.
• Must have BLS provider certification through the American Heart Association.
• Previous experience with psychiatric care preferred.
• Excellent communication skills and exceptional level of professionalism.
• Strong organizational skills and ability to adeptly work with multiple computer systems simultaneously.
• Bilingual in Spanish/English a strong plus!!
