Geographic Area:

Resurrection Home Health Services is hiring Occupational Therapists for Chicago and Suburbs, all zones, on registry basis (as needed).

Duties & Responsibilities:

• Evaluate and establish plan of care for home care patients in accordance with the principles and practices of occupational therapy in accordance with the patient's age, developmental level and diagnosis.
• Administer and instruct patients in prescribed occupational therapy treatments to restore function and maximize independence with activities of daily living.

Qualifications:

• Bachelors degree in Occupational Therapy.
• Current Illinois license in Occupational Therapy.
• CPR certification.
• Valid driver's license & proof of auto insurance.
• 1-2 years OT experience.
• Good communication skills.
• Must be able to provide own transportation to patients' homes.
