Position Description
Master’s degree preferred, preferably in Human services or Social Work, previous case management and nursing experience highly desirable.
Bilingual (Spanish) preferred.
Experience working in a community based medical environment a plus.
1-2 years experience working in a community service environment with children or pregnant women, including breastfeeding support.
1-2 years in conducting health education seminars/workshops
A working knowledge of Illinois Department of Human Services (IDHS) and the services they provide.
1-2 years of prior case management experience.
Current Illinois state driver’s license required; reliable transportation.
Track all prenatal standards of care and maternal-child outcome data
To sort and track all newborn tracking sheets for initial appointments
Prepare monthly and or weekly reports
Provide education on prenatal preparations for breastfeeding
Daily message retrieval (except Sunday)
Maintain privileges at FFHC partnering hospitals to allow for access to mother-infant and to provide lactation support
Daily rounds Mon-Fri in-person at partnering hospitals. This visit will be
o To collect pertinent data
o To support lactation
o To facilitate insurance assignment of infant; and
o To facilitate appointments back into FFHC for infant and mother
Daily telephone follow-up calls (except Sundays) to mothers who deliver outside FFHC’s partnering hospitals or any deliveries after Friday business hours
Serves as on-site lactation resource
Other duties as assigned
