Geographic Area:

Resurrection Home Health Services is hiring a full-time physical therapist for zones 13 & 14(North-Madison; South-City limits; East-Lake Michigan; West-Harlem)

Duties and responsibilities:

• Plan and implement specific treatment/programs for home health patients according to principles and practices of physical therapy.
• Evaluate patients' functional ability using appropriate tests.
• Develop specific physical therapy goals to be included in the Plan of Care.
• Administer and instruct prescribed physical therapy treatments to restore functions, relieve pain, and prevent further disease or injury.

Qualifications:

• Bachelor's degree in Physical Therapy.
• Current Illinois license in physical therapy.
• CPR certification.
• Valid Illinois driver's license & proof of current auto insurance.
• 1-2 years experience.
• Good communication skills.
• Extended periods of standing and walking; ability to lift and move 5-100 lbs and bending during work shift.
• Must be able to provide own transportation to patients' homes.
