Position Description

Position Summary: Provides medical care to adult, adolescent and pediatric individuals and families in the Health Center & other facilities, and in the home when appropriate. Plans and implements programs in community health assessment and education. Collaborates with the administration, medical staff and nursing staff of network centers in setting standards for overall care and implementation of these goals. Directly supervised by the Medical Director or other designated physician but works independently as appropriate for training and experience.


Essential Duties and Responsibilities:
1. Obtains and records complete medical, family and social history.
2. Performs general and physical exams, including routine pelvic exams.
3. Orders routine lab tests & x-rays and interprets the results.
4. Makes assessments based on the above.
5. Formulates a health care plan.
6. Makes referrals to Internists, Pediatricians, Family Practitioners and/or Obstetricians in Health Center and to specialty referral sites, according to protocol.
7. Provides care for acute, self-limited diseases and uncomplicated chronic diseases, as well as, family planning.
8. Provides routine preventive measures and screenings, as well as, certain aspects of patient education and counseling.
9. Provides continuity of care to a panel of patients.
10. Participates in the Health Disparity Diabetes collaborative.
11. Provides follow-up for a panel of patients.
12. Follows patients through hospitalizations with some routine contact.
13. Participates in planning and implementation of community assessment, outreach health education, health education seminars, health fairs, and other activities involving provider staff.
14. Participates in Quality Improvement activities of the health center.
15. Acts as a participant & resource for nursing and other staff in training and other staff conferences, meetings and activities.
16. Adheres to NNHSC team productivity guidelines of 2100 patient visits per year.

Non-Essential Duties and Responsibilities:
1. Maintains and distributes literature to patients.
2. Participates in at least one health fair annually.
3. Represents NNHSC at community meetings, providing in-service training on health issues as needed.
4. Maintains and promotes customer relations standards.
5. Performs other duties as assigned.

Education: Master’s prepared as a Family or Adult Nurse Clinician.

Required Licensure and/or Certification: Currently licensed as an Advanced Practice Nurse in the State of Illinois.

Minimum Experience: Experienced in working in adult medicine in a significantly independently setting managing a range of different problems.

Other Requirements: Saturday hours, evening hours and/or may be required to work at a different site.
