Position Description
Position Summary: Participates in the planning and delivery of health care to patients (adults & geriatric) during clinic sessions under the direction of the RN clinical supervisor and the Charge Nurse. May supervise medical assistants, nurse’s assistants and receptionist. Exhibits professional attitude toward patients, visitors, providers and co-workers.
Essential Duties and Responsibilities:
1. May serve as team leader for the clinical area, coordinates staffing and health center needs with other team leaders, trouble shoots with providers to improve clinic function and flow.
2. Prepares assignments for clinic generalists.
3. Collects statistical information.
4. Monitors supplies/inventory and prepares supply list.
5. Maintains policy and procedure manual in the clinical area.
6. Participates on health center Committees as assigned.
7. Participates in the orientation & training of new staff.
8. Participates in the evaluation of clinic generalists and clerk receptionist.
9. Assist with patient/client check –in and exits. Takes vital signs (obtaining weight & height), document chief complaint (including health histories) and assists patients in completing histories form and other forms.
10. Makes appointments for specialty consultations, diagnostic tests and emergency referrals.
11. Participates in the public awareness and educational presentations in the community.
12. Performs follow-up on abnormal lab tests and missed appointments.
13. Participates in the development and implementation of the patient education curriculum for the clinical area.
14. Collaborates with other team leaders in the maintenance of the procedure rooms and lunch schedule.
15. Provides and documents nursing care for patients, including but not limited to, treatments and administration of medication.
16. Adhere to HIPAA guidelines/policies.
17. Obtains specimens.
18. Maintains current Licensure
19. Attend department meetings.
20. Practices Universal pre-cautions.
21. Maintains patient and health center confidentiality.
22. Participates in chart audits.
23. Triages walk-in patients, and telephone calls from patients and document triage advice in accordance with the triage policy.
24. Adheres to established NNHSC policies & procedures.
25. Documents all care provided.
26. Serves as a team leader for the clinical area
Non-Essential Duties and Responsibilities
1. Participates in the development and implementation of educational programs for patients and community groups.
2. Collaborates with team leaders in other clinical area towards appropriate utilization of available staff and educational resources.
3. Compiles monthly statistical information and communicates to the Director of Nursing and the Medical Director.
4. Identifies potential problems and utilizes a preventative approach to avert disruption.
5. Assumes primary responsibility for the implementation and monitoring of the appointment system in assigned clinical area.
6. Maintains and promotes customer relations standards.
7. Performs other duties as assigned.
Education: High School Diploma or equivalent. Graduate of an accredited Practical Nurse Program.
Required Licensure and/or Certification: Current LPN License in State of Illinois.
Minimum Experience:
Supervision: May supervise Clinic Generalists and Clerk Receptionist.
Other Requirements: Saturday hours, evening hours and/or may be required to work at a different site.
