Position Description

Position Summary: Provides and evaluates WIC nutrition services to individuals, families and community groups. Contributes in the effort to achieve the management and nutrition education objectives set for the fiscal year.


Essential Duties and Responsibilities: (Use separate sheet if necessary)
1. Assists Director, Coordinator and Site Supervisor in meeting assigned caseload set for fiscal year.

2. Assists Director, Coordinator and Site Supervisor in meeting the program goals and objectives set for fiscal year. (e.g. breastfeeding, immunization, FCM integration, Early Entry etc.)

3. Provides dietary assessment and counseling to individuals or high-risk clients. Formulates care plans and documents interventions using the SOAP format. Meets the recommended number of 300 patients seen per month.

4. Provides secondary contact for high-risk clients and follow-up individual or group education sessions for clients.

5. Enrolls in necessary training, e.g. breastfeeding management and support, CHP training, Cornerstone training and Special Formula training.

6. Conducts age-appropriate, interactive-group nutrition education activities such as games, coloring activities, food demonstrations etc.

7. Demonstrates ability to run, analyze, evaluate, and interpret, Cornerstone reports and other WIC surveillance data. Demonstrates ability to apply knowledge of function reports to improve delivery of WIC services. Submits weekly and monthly reports on time.

8. Participates in special events that will increase the awareness of the role of nutrition in the prevention of chronic diseases, and in the promotion of health e.g. Nutrition month, World Breastfeeding Week, Diabetes Week, Men’s Health Week, and AIDS Awareness Week etc. Conducts special projects.



9. Conducts patient and staff satisfaction surveys, surveillance data etc.

10. Maintains good attendance record. Reports to work punctually..

11. Demonstrates a positive relationship with co-workers, supervisor and clients. Maintains professional attitude.

12. Maintains appropriate dress code, wears ID badge and appropriate uniforms. Demonstrates prudent use of phone and supplies. Attends meetings. Performs related duties as required.

13. Adheres to NNHSC’s Corporate Compliance Program.


Non-Essential Duties and Responsibilities:

1. Performs clerical work in the absence of clerical staff.
2. Organizes and maintains files.
3. Maintains and promotes customer relations standards.
4. Performs other duties as assigned.


Education: Bachelor’s Degree with a major in foods and nutrition.


Required Licensure and/or Certification: LDN or Registered Dietitian Certification
preferred. Dietetic training/internship
preferred.


Minimum Experience:


Other Requirements: Saturday hours, evening hours and/or may be required to work at a different site.
