Position Description

Position Summary: Provides Case Management Services to pregnant and parenting women, infants and children to effect the delivery of health and supportive services in an integrated manner.


Essential Duties and Responsibilities: (Use separate sheet if necessary)

1. Maintains an assigned caseload.

2. Conducts monthly or as needed Home Visits to each participant.

3. Completes Individual assessments to determine the level of the client’s health & service needs.

4. Service planning (i.e. development of care plans) and resource identification as applicable.

5. Links client to needed services; monitors service delivery and client’s compliance.

6. Documents client’s encounters and data (Includes electronic health record and cornerstone systems documentation).

7. Participates in community workshops, conferences, seminars, school programs and health fairs.

8. Participates in client staffings and case conferences, as appropriate.

9. Participates in identified staff development activities.

10. Submits written weekly and monthly reports.




Non-Essential Duties and Responsibilities:

1. Client identification through assigned outreach activities.

2. Advocates on client’s behalf to facilitate access to services.

3. Coordinates or facilitates small health education workshops.

4. Performs other related duties as assigned.


Education: Associate’s Degree (in a behavioral science, social service or health-
related area).


Required Licensure and/or Certification:


Minimum Experience: 2 years experience in community health or maternal &
child health OR 1 year experience in child, family or
community services. Good organization and problem
solving skills. Typing and/or basic data entry skills.
Proficiency in basic counseling skills. Knowledge of
community resources and area service providers. Able to
read, write and speak English. Good oral and written
communication skills required.


Supervision:


Other Requirements: Saturday hours, evening hours and/or may be required to work at a different site.
