Position Description
Position Summary: Provide substance abuse treatment and counseling. Perform substance abuse assessments, treatment planning, record management, and/or case management for patients referred into treatment for alcohol, drug and/or nicotine abuse/dependency in accordance with DASA Licensing Regulations and ASAM Patient Placement Criteria.
Essential Duties and Responsibilities:
1. Conduct weekly drug and/or alcohol screenings to promote the early identification of substance abusers and facilitate treatment referral for NNHSC patients.
2. Conduct annual substance abuse evaluations and assessments on all patients enrolled in the EIS program.
3. Provide substance abuse evaluations, assessments and screenings for referred patients.
4. Develop individual treatment plans for each enrolled patient within 14 days of admission. Maintains an ongoing review of service plan and keeps up to date casenotes on each participant enrolled in the program.
5. Conduct daily group treatment with identified substance abusing patients. Maintain a minimum caseload of 20 individual patients. Conducts outreach when appropriate to meet minimum caseload requirements.
6. Provide individual, group and family counseling or significant others for each patient as needed.
7. Refer patients for urinalysis and drug screening. Provide post detoxification support for patients; provide follow-up and monitoring of patients.
8. Provide general education concerning substance abuse treatment and prevention to NNHSC providers, staff, patients and community residents as needed.
9. Report/summarize program activities for weekly and monthly reports. Submit reports in a timely manner.
10. Prepare data for daily DARTS reporting and enter DARTS weekly into the reporting system.
11. Participate in case staffing and case conferences with the Mental Health team, Case Management, and other NNHSC programs/providers as appropriate
12. Ensure that all guidelines, regulations and reporting requirements pertaining to the Substance Abuse Treatment program are in compliance with DASA Licensing Regulation 2060, ASAM criteria and departmental policies.
13. Meets the Certification Boards CEU requirements in order to maintain Board Certification (20 Board approved CEU’s per year).
14. Collaborate and maintain a working relationship with NNHSC Medical providers and other programs/providers at NNHSC.
15. Adhere to NNHSC Confidentiality policy and HIPAA regulations.
16. Adhere to policies and regulations set forth by funders or collaborate partners.
17. Perform other duties as directed.
Non-Essential Duties and Responsibilities:
1. Ensure that all patient records are safeguarded and stored in compliance with NNHSC standards, HIPAA regulations, and applicable DASA licensing regulations.
2. Provide referrals to local C.A., N.A., A.A., Alanon, and Alateen for patients as needed.
Education: Bachelor’s Degree, Master’s preferred and/or formal training in the field of Substance Abuse Treatment, rehabilitation or counseling.
Required Licensure and/or Certification: Certified Alcohol and other Drug Abuse Counselor (C.A.D.C.) through the Illinois Alcohol and other Drug Abuse Professional Certification Association, Inc. or be board eligible for provisional certification by having an application portfolio board approved by the Illinois Alcohol and other Drug Abuse Professional Certification Assoc., Inc.
Minimum Experience: Minimum two years experience counseling, assessing, treating or rehabilitating substance abusers and/or alcoholics, preferably both, in an environment where cultural sensitivity was required, particularly among minority groups and where being economically, as well as, educationally disadvantaged, were major substance abuse factors.

Other Requirements: Saturday hours, evening hours or work at a different site may be required.
