The Telehealth and Disease Management Coordinator is responsible for establishing clinical practices that support evidenced and best practices and associated protocols in managing chronic diseases in Home Care Services. This coordinator is also responsible for establishing clear protocols for the use of telemonitors, trend data to demonstrate the value of telehealth and its return on investment and research industry trends in telehealth and related technology to strategically position the division for future care delivery. This Coordinator has matrix responsibility over employees who manage care delivery utilizing telemonitors.
Knowledge, Skills and Abilities Required:
· Master's degree with demonstrated experience in disease management. Advanced Practice Nurse (APN) preferred.
· Current nursing (RN) license in the State of Illinois.
· Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Maintains a valid driver's license.
· Demonstrates ability to work independently and execute own workload.
· Must be able to use and operate medical and/or office equipment.
· Excellent interpersonal skills to communicate effectively.
· Experience in developing disease management protocols or pathways preferred.
· Minimum of 2-3 years of progressively more responsible related work experience in nursing.
· Expected to take department leadership calls on a rotating basis, as required.
