Position Description

OSF Home Care Services is looking for a motivated, skilled and forward thinking Physical Therapist to add to our team of PTA's, OT's and COTA's, SLP, MSW and Nursing professionals in our Pontiac, IL region. As one of our Physical Therapists, you will plan the care of patients. You will be responsible for the provision of therapeutic evaluation, exercises and treatments to restore function and prevent disability following disease, injury, or loss of a body part under MD orders, and in accordance with the State Practice Act.

This is a full time, 40 hours a week flexible position. You can set your own hours, working 5 - eight hour days or 4 ten hour days or any combination you choose to work between Monday and Saturday.

Requirements: Graduate of an accredited college of physical therapy; Current license to practice as a Physical Therapist in the State of Illinois; Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Maintains a valid driver's license; Minimum of one year recent physical therapy experience preferred; Home healthcare experience preferred; Clinical affiliation or experience in comparable hospital setting desirable; Expected to take night and weekend calls, and make home visits and/or work in office during the evening, night, weekend and/or holiday hours as required; Must be able to use and operate all physical therapy equipment and/or office equipment, including Point of Care devices.
