Position Description

OSF Home Care Services has a current opening for a Hospice Medical Social Worker in our Pontiac, IL regional office.

The Medical Social Worker is responsible for identifying socioeconomic and emotional factors that exist which are or can be an impediment to the effective treatment of the patient’s medical condition or rate of recovery. Initiates a plan of treatment to reduce or alleviate problems to insure the best possible care of the patient. Serves as liaison between the home care department and the community to integrate services, coordinate care for patients and enhance good working relationships. Is responsible for Medical Social Service case coordination and supervision of the clinical activities of social work assistant. Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. This requires that the individual demonstrates knowledge of the principles of growth and development as well as the physical, emotional and psycho-social needs of the patient population served. Accountability to the Care Manager in relation to Utilization Processes and PI/Education Coordinator in relation to regulatory and compliance standards.

Knowledge, Skills and Abilities Required:

· Master’s degree in Social Work or related field and 1 year of experience. Current licensure by state in which agency services.

· Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Maintains a valid driver’s license.

· Demonstrates ability to work independently and execute own workload.

· Working knowledge of human behavior/development and theories of social work process.

· Must be able to use and operate medical and/ or office equipment, including Point of Care devices.

· Previous experience in medical social work with home care preferred.

· Expected to take night and weekend calls, and make home visits and/or work in office during the evening, night, weekend and/or holiday hours as required.

· Accountability to the Care Manager in relation to Utilization Practices.
