Job Description	QUALIFICATIONS: Current Illinois license as a Registered Nurse or license eligible required. BSN and one year of nursing experience desired. Experience in Hospice/Home Care preferred. ESSENTIAL FUNCTIONS: Directs patient care provided by support personnel for a specific shift. Identifies changes in patient's physiological condition. Communicates patient's status to multi-disciplinary team members. Communicates directly with patients and families about nursing care. Provides clinical interventions to meet patient's needs. Documents nursing process. Assimilates information from multiple sources and implements appropriate action plan to provide safe, quality care. Assures safety and security of patients, significant others, and staff. AA/EOE
