Kindred Healthcare and Palomar Health have signed a Joint Venture and
will open a new acute inpatient rehabilitation hospital in Spring 2021.
The two-story, 58,000-square-foot facility will feature 52 inpatient beds and is located on the campus of Palomar Medical Center in Escondido, California. The new hospital will offer treatment services to a variety of rehabilitation patient types, including brain injury and stroke patients. This will be San Diego County's first standalone rehabilitation hospital. Expected to open in 2021, this facility comes as Palomar and Kindred continues to run its existing inpatient rehabilitation program, which has been run by Kindred since 2000.
ESSENTIAL FUNCTIONS:
Job Specific:
Responsible for staff orientation andongoing education to improve QI score accuracy at admission, throughout thestay and upon discharge. Serves aseducator for Quality Indicators (QI) scoring according to Uniform Data System(UDS) training materials.
Serves as a resource to licensedclinicians and clinical staff regarding questions and interpretation of QIscoring guidelines, according to UDS training guidelines, in order to assurethe accuracy of the QI scores.
Provides ongoing education to the staffof the hospital to assure accurate and timely completion of the IRF-PAI.
Responsible for staff orientation andongoing education and facilitate biannual UDSMR staff credentialing for QIscoring.
Review medical record documentationusing the medical record including UDS Pro-Doc system to support the accuracyand timeliness of the functional scores and comorbid conditions, according toCMS guidelines.
Ensures that the IRF-PAI is consistentwith the medical record documentation and reflects the functional status and 24hour care needs of the patient.
Collaborates with physicians andinterdisciplinary team members to ensure the most accurate assessment of theneeds and strengths of patients are captured in the UDS Pro-Doc system and inthe medical record.
Review current facility clinicaldocumentation in the medical record and UDS Pro-Doc and evaluate accuracythrough the use of QI Manager.
Compares actual knowledge of thepatient to care provided to the patient and the information contained on the IRF-PAI. Makes appropriate corrections to the IRF-PAIin collaboration with the interdisciplinary team in order to accurately reflectthe burden of care upon admission, throughout the stay and upon discharge.
Investigates the Impairment Group CodesIGC and co-morbidities in collaborative manner with the physicians and codersthat would allow for accurate coding of the patient's stay.
Oversees/completes and analyzes and QIdata ensuring the accuracy and the completion of the Inpatient RehabilitationFacility-Patient Assessment Instrument (IRF-PAI) according to CMS guidelinesand prior to electronic transmission.
Reviews the medical recorddocumentation to ensure accuracy and to support the correct Case Mix Group(CMG) assignments
Identifies target reference dates forthe completion of the IRF-PAI for all patients.
Coordinates the completion of theadmission assessment of the IRF-PAI by the fourth day of the patient's stay andthe third day following discharge for each inpatient rehabilitation patient. May require discussions/interactions withvarious staff members concerning differences in reported and/or medical recorddocumentation of functional status to complete the PAI. Uploads QI scores in UDS Pro-Doc necessary tocomplete the IRF -PAI.
Coordinates and collaborates with HIMdepartment to ensure timely completion and transmittal of the IRF-PAI to CMSnational database.
Maintains current knowledge of the UDSPro-Doc system and CMS IRF-PAI software and provides expertise on the softwarewithin the inpatient rehab setting and contributes to facility processimprovement plans.
Maintains updated and ongoing knowledgeof the regulations concerning the IRF-PAI and its completion within theinpatient rehab setting.
Has working knowledge of capabilities ofUDS Pro-Doc, including various reports and how to access and use.
Has working knowledge of UDSPro-central, including various reports and how to access, interpret and use.
Serves as an information source andeducator to IRF-PAI and UDS users.
Periodically performs medical recordaudits including audits of the IRF-PAI instrument, clinical documentation andbilling information to ensure accuracy and consistency of documentation.

Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program or Licensed/Registered PT, OT, SLP
