Overview

SST CIMs provide scribe services for multiple hospitals in an Emergency Department or inpatient unit at the direction of a Clinician (Physician, Nurse Practitioner or Physician Assistant) allowing the Clinician to spend more time with the patient, work more productively, and produce accurate and thorough documentation in real-time. An SST CIM does not act independently, but documents the Clinician's previously determined dictation and/or activities in the electronic medical record (EMR). SST CIMs travel to multiple client hospitals to provide CIM coverage during staffing shortages or growth periods as directed by Management. SST CIMs train newly hired CIMs and mentor CIMs during their introductory period, and as requested. SST CIMs may also participate in departmental or company-wide projects on-site or at the corporate office when advanced CIM services are required.

Responsibilities

Essential Functions of Job:

    Provide scribe services for multiple Emergency Departments/Inpatient Units at the direction of a Physician or Advanced Practice Provider
  • Listen and take notes while accompanying the Clinician to the patient room or receiving verbal dictation
  • In the EMR, document accurate, thorough and timely physician notes that reflect the full complexity of care provided:
    • Document patient care activities performed by the Clinician (i.e. procedures, medications, conversations with consultants)
    • Document applicable information from various sources (i.e. hospital information systems, prior visits, nursing triage notes, EMS run sheets)
    • Maintain awareness of conversations regarding patient care between the Clinician and other healthcare clinicians, patients, family, etc. to ensure applicable information is documented
    • Prepare discharge instructions or electronic prescriptions for Clinician review and signature
  • Ensure physician notes meet clinical documentation requirements for risk management, billing, etc.
  • Inform the Clinician of any incomplete documentation and seek out the information to complete the note

  • Prompt the Clinician when the note documentation is ready for review and signature
  • Monitor the status of the patient's course, troubleshoot delays, and keep the Clinician informed of pending items (i.e. waiting for x-ray to be done, all lab results back, etc.)
  • During downtime, offer to help other CIMs or Clinicians with physician note documentation
  • Place phone calls to private doctor's offices, pharmacies, etc. to seek out information as requested by the Clinician
  • Assist the Clinician in navigating the EMR
  • Identify and anticipate client (Clinician) need and respond in an effective manner
  • Train CIMs during the two-week on-site training period or at the corporate office as needed
  • Mentor / coach CIMs on site during the 90-day introductory period or as requested
  • Draft introductory performance evaluations in absence of a Lead CIM as requested
  • Interview CIM candidates as requested
  • Responsible for adhering to Information Security Policies and ensuring Envision is as secure as possible.
  • Perform other duties as assigned
Qualifications

Knowledge and Skills:

  • Excellent written, verbal, and interpersonal communication skills
  • Successfully demonstrated proficiency as a CIM/Scribe
  • Successfully demonstrated strong client service skills
  • Strong understanding of the clinical operations of the Emergency Department

Education and Work Experience:

  • A minimum of 6 months experience as a CIM or Scribe
  • Bachelor's degree preferred
  • Experience training/mentoring CIMs/Scribes preferred
  • Experience working at multiple ED or Hospitalist sites preferred
  • Experience working with EDIMS, Cerner, and Epic EMRs preferred
