Job Description

Lutheran Social Services of Illinois is seeking an energetic person to join our team as aCounselor/Case Managerin our Screening, Assessment & Support Services (SASS) program on the Northwest side of Chicago.


Duties and Responsibilities:

  • Provide short-term, community based individual, family, or group therapy to a caseload of clients as determined by the SASS program needs
  • Complete IMCANS assessment in order to formulate an accurate diagnosis and comprehensive treatment interventions to serve the identified client and family
  • Participate in back up on-call rotation Monday-Friday to conduct crisis, on-site assessments of clients to determine the need for inpatient psychiatric hospitalization.
  • Develop close working relationships with DCFS
  • and DMH funded agencies, and other child and adolescent mental health
  • service providers throughout the northwest side of Chicago
  • Provide intensive outreach in an attempt to engage clients who do not follow through with post discharge/deflection appointments
  • Maintain positive and mutually respectful relationships with
  • other human service providers, entitlement agencies, health care vendors,
  • and the community
  • Provide case management services as defined in the treatment plan that coordinate, support, and advocate for clients who need multiple services and require assistance in gaining access to and in using mental health, social, vocational, educational, child welfare, and other community services and resources


Qualifications:

  • Masters' degree in social service field required
  • LSW/LPC preferred.
  • BilingualEnglish/Spanishrequired.
  • Valid driver's license and car insurance/car required.
