Job Description
Overview

Expected pay for this position is $37.99 - $44.70/hour. Actual pay will be determined by experience, skills and internal equity.

POSITION SUMMARY:

The Supervisor of Case Management is responsible for effectively providing daily leadership for Case Management functions at assigned Medical Center(s). Responsible for assessment, developing and implementing the care processes for care coordination, transition of care, and discharge planning in collaboration with frontline staff, team leads, facility leaders, providers, and regional Case Management leadership. Ensures the services and programs provided by the staff achieve established process and outcome measures. Responsible for consistent application and follow through with established policy and procedure around Case Management functions as outlined by Ministry Case Management. Utilizes leadership skills and competencies to create and enhance a culture that supports Case Management. In accordance with strategic initiatives, incorporates effective change management philosophies in all performance improvement efforts. Focuses on consistent performance of staff, supports a proactive approach for length of stay management, outlier management, and escalation of barriers.


Qualifications

REQUIRED QUALIFICATIONS:

Education:

Master's degree in social work

Experience:

3 years of experience caring for patients chronic, complex illnesses adult and/or pediatric care management.

Licensure/ Certification:

Clinical Counselor License or Clinical Soc Worker License.

All MSW Mission Partners must obtain LCSW or LCPC licensure within 1 years from date of hire.

Case Management certification for adult or pediatric as appropriate within 3 years of hire.

Other Skills/ Knowledge:

Excellent interpersonal and communication skills.

Solid computer skills, including proficiency with Microsoft software.

Strong analytical and problem-solving skills, with the ability to be detail oriented.

Broad knowledge of chronic illness and medical and community resources.

PREFERRED QUALIFICATIONS:

Licensure/ Certification:

License Social Worker Case Management Certification

Other Skills/ Knowledge:

Interpersonal skills in order to coach, mentor, and effectively lead assigned staff, facilitate, organize, and direct the work of others and to maintain positive relationships among leadership, Ministry and facility staff, and internal and external stakeholders.

Positive, empathetic approach and ability to communicate effectively to patients and families with a variety of health issues, age groups, and socioeconomic backgrounds.

Proven skills in written and verbal communications, problem solving, decision making, independent judgement, creativity, prioritization, and delegation.
