LSSI is offering a part-time position for RN/Nurse Educator, to provide nursing services focused on health promotion & disease monitoring to Colbert and Williams Class Members residing in identified facilities and the community. Coordinate with Psychiatrist/ APN/ Case Managers/ Medical physicians/ etc.


Duties and Responsibilities:

  • Provide health promotion education to clients and guidance to staff.
  • Medically screen clients to provide disease management in accordance with the psychiatrist/APN/medical physicians.
  • Provide direct nursing services to clients for health history, weight, BMI, etc, in order to safely facilitate their transition into independent living.
  • Coordinate health information exchange with psychiatrist/APN/medical physicians/MCOs in accordance with privacy laws.
  • Maintain documentation in accordance with agency/regulatory standards.
  • Transportation of clients in personal vehicle and agency vehicle required.

Qualifications:
  • RN required & proven nursing skills.
  • Excellent verbal & written communication skills.
  • Basic computer skills.
  • Must be willing to work with clients in their home/community.
  • Ability to engage & motivate persons with a mental illness.
