Job Description

1. Assembles record in correct order, verifying that all materials belong to the named patient, and placing the record in the correct folder and sequence.

2. Analyzes chart to identify any designated deficiencies, flagging orders and entering deficiencies into database, and printing/attaching deficient reports.

3. Prints and attaches deficiency sheet to the front of the visit.

4. Assists the physician in clarifying deficiencies, answering questions and following up on issues to ensure completion and good customer service.

5. Reanalyzes charts following a physician visit, entering changes to deficiencies and other data fields to reflect pending documentation.

6. Maintain delinquency reports and physician letters, and communicate weekly to physicians and hospital leadership.

7. Communicates form changes to analysis team and HIM management.

8. Performs an audit on Friday to locate any discharges not previously returned to the department from the inpatient units.

9. Performs audits as defined by the HIM Committee and presents data in an accurate, timely fashion. Performs data quality checks on system to ensure data is downloaded to system on a nightly basis.

10. Cross-trained to perform duties of absent staff.

11. Performs other duties and special projects as assigned by manager.

Knowledge, Skills and Abilities:

1. High School Diploma or equivalent. One to two years experience in health care information management. General clerical skills including typing and filing. Computer experience to include Windows, Word and Excel. Experience with HBOC and Epic a plus. Knowledge of medical terminology desirable.

2. Ability to handle multiple projects and support multiple systems. Ability to appropriately prioritize tasks. Works independently. Consistent, responsible decision-making skills. High level of organizational skills.

3. Ability to stand, stoop and lift.

4. Ability to deal effectively with all Children's Hospital of Chicago Medical Center staff at all management levels and external staff. Demonstrates excellent communication skill necessary for interaction at all levels.

5. Data analysis and report skills.
