Position Highlights:

  • Hours: PM Shifts


A Brief Overview:
Provides direct social work assessment and services to patients and families. Shall act in accordance with hospital and department philosophy, the NASW code of ethics, federal, state, and local laws, and other regulatory bodies such as HFAP.

What you will do:

  • Demonstrates a commitment to the mission of Swedish Covenant Hospital and demonstrates a service orientation and adheres to all responsibilities and standards of the Hospital.
  • Performs psychosocial assessment/evaluation of patients and significant others
  • Implements a plan of care to attempt to resolve psychosocial problems.
  • Completes and maintains department records and statistics. Documents case activities in
  • the medical record.
  • Responds to off hour emergency calls, as required for consistent delivery of services.
  • Participates in education for patients and significant others.
  • Acts as patient advocate and resource person to staff and the community on social work issues.
  • Maintains updated resource information for the department.
  • Participates in providing New Employee Orientation
  • Provides supervision to MSW Interns and LSW staff
  • Provides In-Services/Education for the department
  • Performs other related duties as assigned or requested.


What you will need:

  • Masters Degree Social Work Required
  • 1 Year of social work experience in healthcare setting preferred.
  • Ability to work with interdisciplinary personnel, patients and families.
  • Manages assigned caseload responsibilities.
  • Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR) Required
