Job Description:

I. GENERAL SUMMARY:

Reporting to the Director of QMI, with accountability to the QI Team and selected strategic performance improvement teams, acts as a quality professional to objectively and systematically assess, monitor, evaluate and improve the processes that affect the quality of patient care by supporting and coordinating selected Medical Staff and strategic quality improvement activities based on key strategic initiatives.

II. REPRESENTATIVE FUNCTIONS:

Assists in the design and selection of clinical performance measures, collects data, conducts case review, generates reports, prepares analysis highlighting variations in clinical process and outcomes, identifies trends and opportunities for improvement, and disseminates information to respective audiences. Utilizes the appropriate quality management information system for data needs and presentation. Integrates data from other sources as needed. Performs research/literature searches on suggested clinical topics for review.

Utilizes statistical analysis methodologies to identify opportunities for improvement or to measure effectiveness of interventions.

Serves as the database administrator for selected quality management information systems.

Coordinates and facilitates the physician peer review process through the Medical Staff quality committee structure. Provides objective rate-based physician quality performance information to the medical staff for the purpose of reappointment.

Coordinates the preparation for selected meetings, including agenda and meeting materials preparation, room reservations, refreshments, etc.

Coordinates, in collaboration with the physician committee chairman and/or QI Team Leader, any follow-up action to be taken on identified quality improvement issues to improve outcomes and integrate best practices. This is accomplished via ad hoc meetings, face-to-face interactions or written communication.

Based on clinically valid criteria, identifies and communicates referrals to Risk Management, Infection Control and others as indicated.

Prepares annual reports for selected areas of responsibility.

Keeps abreast of current trends in the field of quality improvement and serves as a resource on quality improvement standards and methodology. Is knowledgeable of and shares knowledge regarding such.

As an active member of the QI Team, assists in the development and attainment of team goals, supports team members, and shares involvement related to strategic activities on a regular basis. Accountable for meeting team goals.

Adheres to all Northwest Community Hospital standards, policies, and procedures.

III. SPECIAL SKILLS AND ABILITIES REQUIRED:

Observes strict confidentiality of all information pertaining to quality improvement activities and practices discretion and sound professional judgment at all times.

Possesses excellent written and verbal skills necessary to communicate effectively with all levels of the organization.

Exhibits skill in analytic fundamentals to include the application of basic statistical processes, data analysis and reporting and computer use.

Possesses the ability to use automated methodologies for the storage and retrieval of data.

Exhibits skill in creating a work environment that foster team work, emphasizes quality and promotes learning.

Ability to work independently and as part of a group.

IV. KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

Clinical background in healthcare, the equivalent of at least 4 years of college education i.e., RN, an experienced RHIA (Registered Health Information Administrator) or RHIT (Registered Health Information Technician) degree.

Minimum of 2 years experience in health care quality management.

Basic knowledge and understanding of QI processes, tools and problem solving methodology, i.e. FOCUS-PDCA.

CPHQ certification is preferred.

Overall Qualifications:

Masters Degree, Masters Degree: Nursing
