What you will need:

  • Graduated from ACPE accredited Doctor of Pharmacy degree program
  • Has submitted a Curriculum Vitae and an official transcript from an accredited School / College of Pharmacy
  • Should be educated regarding IV admixture, drugs, disease states, pharmacology, infection control, and pharmacy law demonstrated by completion of rotations with a clinical focus and grades/ preceptor comments from those rotations
  • Has submitted three Letters of Recommendation from preceptors on clinical rotations
  • Has submitted a Letter of Intent including a statement of professional goals and reasons for pursuing the Pharmacy Practice Residency
  • Needs to have excellent verbal and written communication skills, as well as an ability to work well with others
  • Pharmacy technician or pharmacist work experience is preferred.
  • Has an IL pharmacist license or is eligible for licensure in IL and will obtain it in first month of residency
  • Participated in and obeyed the rules of the Residency Matching Program
  • Demonstrates an interest in and aptitude for advanced pharmacy training
  • Participates in pharmacy organizations (ASHP and others)

LICENSE/CERTIFICATION

  • Currently or soon to be licensed by the State of Illinois Department of Professional Regulation (of Education and Registration) as registered pharmacist.

JOB SUMMARY

Interprets physician orders, evaluates patient profiles, makes appropriate interventions as needed, enters or verifies computerized medication orders, answers drug information questions, completes consults for Heparin, Warfarin, Insulin, Vancomycin, and Aminoglycoside dosing, rounds with the interdisciplinary medical teams, compounds or prepares medication for patients as prescribed by the physician, and practices and promotes the concept of patient-focused care. He/she participates in Medication Utilization Evaluations (MUE), education, patient monitoring, Quality Improvement (QI) programs, Committee work, and drug information activities of the Pharmacy. He/she is responsible for the completion of a residency project which may be in the form of original research, a problem-solving exercise, or the development, enhancement, or evaluation of some aspect of pharmacy operations or patient care services.

RESPONSIBILITIES

*Demonstrates a commitment to the mission of Swedish Covenant Hospital and demonstrates a service orientation and adheres to all responsibilities and standards of the Hospital.

  • Full-time, 12 month commitment

*Works independently every other weekend and one evening per week providing staffing coverage

Participates in medication utilization evaluations (MUE), education, patient monitoring, Quality improvement (QI) programs, Committee work, and drug information activities of the Pharmacy.

Profiles medication orders via computer accurately, makes appropriate clinical interventions for optimal patient outcomes, and documents the changes.

Provides in-services to medical staff, residents, and nursing staff for appropriate use of medication and safety

Precepts pharmacy students to provide guidance in their activities and to become a role model for new pharmacists.

Supervises and trains pharmacy technicians.

Ensures compliance with patient quality standards and maintains quality assurance documentation.

Is responsible for the completion of a residency project

PATIENT CARE/AGE SPECIFIC RESPONSIBILITIES AND QUALIFICATIONS

Makes appropriate clinical interventions according to patient age, height, weight, sex, and condition for appropriate indications and dosing.

Prepares for and rounds with the interdisciplinary medical teams to ensure optimal drug therapy, to ensure compliance with all SCH P&T policies and drug formulary, and to answer drug information questions.

Monitors drug interactions, drug allergies, ADRs, contraindications, and compatibility issues.

Access To Protected Health Information:

Computer system access is assigned based on manager's access level request and on the minimum necessary as described in this job description. The system administrator or manager of the department that has ownership of the system/module and gives approval for level of access. See the Computer System Access Control Policy.

MANAGEMENT RESPONSIBILITY

Supervising the Pharmacy in the absence of the Pharmacy Supervisor and training Pharmacy Technicians as needed.

INTERNAL AND EXTERNAL CONTACT

Internal - Pharmacists, Technicians, Physicians, Nursing, Other Healthcare Professionals including but not limited to Respiratory Therapists, Clinical Dieticians, PT/OT, etc.), Transportation, and Patients.

External - Pharmaceutical Sales Representatives and other pharmacies or pharmacy schools.

ENVIRONMENT

Spends a majority of work time sitting (70%), with some standing and walking (30%) around the hospital.

Includes some moving or lifting of objects (up to 30 lbs.) on a regular basis (0.5%), especially bins of checked IV admixtures.

Includes about 65-70% of time using a computer keyboard and communicating with other health care professionals regarding physician's orders and prescriptions.

Exposed to possible injury from chemicals, oncolytic, needles, or equipment (20%).

Exposure to airborne germs when doing clinical activities, renal dosing, or patient counseling on the floor (50%).

Other environmental conditions might include eyestrain and exposure to other computer side effects, such as radiation and high-pitched noise. Also, he/she may be exposed to constant noise from printers and phones, etc. about 50% of the time.
