LSSI is hiring afull-timeAddictions Counselor Case Manager.In collaboration with a child welfare partner, provides intensive, home-based clinical and case management services to women referred by DCFS who have given birth to a substance-exposed infant. Other referrals may be made of families with significant drug usage indicating risk of harm to affected children. Services include substance abuse assessments, treatment planning, individual counseling, referral/linkage, crisis intervention, drug screens and other advocacy. Also works in collaboration with DCFS, Juvenile Court and other community service agencies.

Essential Functions:

  • Provides case management, outreach and aftercare services to adult clients and their families. Typical caseload is 15-20 clients.
  • Facilitates client's admission to inpatient substance abuse treatment or linkage to out-patient treatment, as needed.
  • Facilitates supportive treatment invention to maintain or enhance client's capacity for independent functioning.
  • Intervenes using a holistic approach to change attitudes, beliefs and behavior of clients in an effort to improve clients in an effort to improve client and family functioning.
  • Completes and submits all necessary reports and documentation accurately and on time as required by agency, regulatory bodies and funding sources.

Position Qualifications:

  • Bachelor's degree in social work or a related human services field strongly preferred.
  • At time of hiring must meet certification requirements for CARS and obtainCADC as soon as eligible, no later than within 2 years of hire date.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Background check clearance, both criminal and CANTS.
  • Valid Driver's License and proof of automobile liability insurance required. Access to transportation for visits and to transport clients as needed.
